Congratulations on your appointment to the role of Sports Coach at Pembroke School.

Before completing this form, please read the following information carefully as you will be required to provide evidence of each item to submit this form.

Working with Children Check

In accordance with South Australian child protection laws and School policy, all Sports Coaches are required to provide evidence of a current Working with Children Check (previously known as a Child Related Employment Screening) prior to commencing at the School.

If you hold a Working with Children Check, please provide your Unique ID – this ID reads SRN xxxx-xxxx. If you hold a Child Related Employment Screening, please provide your 6 digit reference number.

Responding to Risks of Harm, Abuse and Neglect Training

All Sports Coaches must complete the Department of Education’s Responding to Risks of Harm, Abuse and Neglect Training. This involves completion of the online Fundaments course and the facilitator-led Masterclass that can be completed online or in person. You need to complete the Fundamentals course before you commence work but you can complete the Masterclass after you commence.

The course may be completed via Plink, the Department’s online learning management system. When you create an account, ensure you create an “SA Independent Schools” account. This will ensure that the right courses are available to you when you login. The course to complete is the “Masterclass and fundamentals for NEW staff or staff with expired certificate: RRHAN-EC (AISSA)”.

First Aid Certificate

Pembroke requires all Sports Coaches to hold a First Aid qualification. This qualification should include the following module:

  • HLTAID009 – Provide cardiopulmonary resuscitation
  • HLTAID010 – Provide basic emergency life support
  • HLTAID011 – Provide first aid

If you do not already hold a First Aid qualification, you may complete this training through the School. The School will cover the cost of this training.

COVID-19 Vaccination Certificate or Proof of Medical Exemption

Under the Emergency Management (Education and Early Childhood Settings Vaccination No 2) (COVID-19) Direction 2021, all individuals working in a school setting must have received a COVID-19 vaccination or have been granted medical exemption. Here is a link to the Service Australia website which outlines what documents you can provide as proof.
